      Firewall Rulebase Change Request Form


Introduction
------------

This form should be submitted to the DePaul University Computer
Security Response Team to request a change on any of the firewalls
managed by CSRT. Do not include sensitive or private information in
this request without employing strong encryption such as PGP. All
submissions must be submitted in standard ASCII text, 72 characters
wide.

All requests require a digital signature be made with a trusted PGP
key signed by a member of the Computer Security Response Team.

Requirements
------------

For any host being protected by CSRT managed firewalls, the host must
maintain current vendor-recommended patch levels and have a patch
routine available for CSRT review. All hosts must conform to CSRT
security guidelines and implement strong security measures. CSRT
RESERVES THE RIGHT TO DISABLE FIREWALL RULEBASE ENTRIES, WITH OR
WITHOUT NOTICE, IN THE EVENT OF A SYSTEM COMPROMISE.
